$5,000 Critical Illness
The AmeriBenefit
Plan provides the primary member (and spouse with the Family
Package) with coverage at up to $5,000 in the case of Critical
Illness. If you (or your spouse) are diagnosed with a life-threatening
Critical Illness, this benefit helps you to concentrate on
getting better, instead of worrying about medical bills.
This plan provides a lump sum benefit if the primary member
(or spouse) is diagnosed with a Critical Illness, including:
life threatening cancer; heart attack; kidney (renal) failure;
stroke; coma; loss of sight, speech or hearing; paralysis;
or severe burns. Waiting periods and pre-existing condition
limitations apply.

The Critical Illness Benefit is underwritten by National
Union Fire Insurance company of Pittsbugh, PA, and is subject
to the terms, conditions, exclusions and limitations of coverage
as set forth in the description of coverage provided in your
membership materials and the Group Critical Illness policy
No. CIP 9600239 issued to AmeriBenefit Plan.
