done by an audiologist, a doctor, or hearing and speech clinic,
send a copy of the audiogram to the Hearing Service at the
address listed below. If you have not had a hearing evaluation,
write in as much detail as you can about how your loss affects
you in specific listening situations. All letters are answered
personally and promptly.
