$2,500 Critical Illness- Family
The AmeriBenefit
Plan provides you and your spouse with a coverage at up to
$2,500 in the case of Critical Illness. If
you or your spouse are diagnosed with a life-threatening Critical
Illness, this benefit helps you to concentrate on getting
better, instead of worrying about medical bills.
This insurance plan provides a lump sum benefit if you are
diagnosed with a Critical Illness, including: life threatening
cancer; heart attack; kidney (renal) failure; stroke; coma;
loss of sight, speech or hearing; paralysis; or severe burns.
Waiting periods and pre-existing condition limitations apply.

The Critical Illness Benefit is underwritten by National
Union Fire Insurance company of Pittsbugh, PA, a subsidiary
of American International Group, Inc.*
(AIG) and subject to the terms, conditions, exclusions and
limitations of coverage as set forth in the description of
coverage provided in your membership materials and the Group
Critical Illness policy No. CIP 9600239 issued to Ameribenefit
Plan.

financial and contractual obligations and support functions
associated with the products issued by National Union Fire
Insurance Company of Pittsburgh, PA, are its responsibilities
