$2,500 Critical Illness
The AmeriBenefit
Plan provides the primary member (and spouse with the family
package) with a coverage at up to $2,500
in the case of a Critical Illness. If you (or your spouse)
are diagnosed with a life threatening Critical Illness, this
benefit can help you concentrate on getting better, instead
of worrying about medical bills.
This insurance plan provides a lump sum benefit if the primary
member is diagnosed with a Critical Illness, including: life threatening
cancer; heart attack; kidney (renal) failure; stroke; coma; coronary
artery bypass graft; loss of sight, speech or hearing; major organ
transplant; paralysis; or severe burns. Waiting periods, eligibility
and pre-existing condition limitations apply.

The Critical Illness Benefit is underwritten by National
Union Fire Insurance company of Pittsbugh, PA, and is subject
to the terms, conditions, exclusions and limitations of coverage
as set forth in the description of coverage provided in your
membership materials and the Group Critical Illness policy
No. CIP 9600239 issued to Ameribenefit Plan.
